[cisco-voip] Calls ignoring line busy state

I also noticed that this is per-device.  I initially thought the per-line
busy would be global, but I set it to 1 for one user and 2 for another, and
both still rang.  It wasn't until I changed the shared line appearance also
that it was fixed.
