[cisco-voip] New dual banked firmware update feature still fails

Last night i scheduled a firmware update to latest 9.2.1 from 9.1.1SR1. The
job schedule is set to run @ 3am where the system will reboot all the 8961
phones.

However, this morning there are some casualty 0.44% out of approximate 700
phones.

After manually reboot the phone, it just blinks the button, after showing
the cisco logo, the screen went blank. I tried to manually specify old
firmware load (9.1.1SR1) on those indivudual phone config, reboot.. still
the same behaviour.

Is there any factory reset key sequence on this new generation of phones? I
tried to google but can't find any.

If there's dual bank (active and invalid load) and it still fails. What's
the dual bank for? Purely for downloading of firmware purpose and not
serving as a fallback upon firmware upgrade failure?
