[cisco-voip] Orphaned Phone Button Templates

Dependency records are best.  There are a few known issues with that. I 
believe it was that dependency records did not display BAT templates 
that used phone templates.

dependency should be first approach.  if you need an SQL approach here 
is a starting point:
run sql select pkid,name from phonetemplate where pkid not in ( select 
distinct fkphonetemplate from device)

it 'looks' right, but I did not validate all scenarios. provided as-is 
with no warranty.
