Just following up, it looks like mid-call features (EFA) does not need an H323 gateway, but without it, you lose the prompts. I've got a case open with the TAC to help me resolve my EFA issues, I'll provide feedback. The TAC engineer did some research and confirmed EFA with MGCP was possible.
Right now, I simply get a busy signal when calling the EFA DID while I have a call on exclusive hold.
I know that you can provide MVA with h323 hairpinning, but I'm not sure if you can provide EFA with hairpinning.

my vague recollection is yes. mid call features depend on the h323 gateway listening in on the call and invoking a tcl or vxml script (increasing fuzziness here).
