Thanks much!
The area was not fiber serviceable by our carrier, hence the DS3. I've got a few others in service where it's as you mentioned, carrier connection straight to my gear, all installed before my company took over, this customer however has their server room nowhere near the Demark.
Also the cards have dropped, 9.3k list now, apply your normal cisco discounts and they are almost affordable....

75-ohm BNC coaxial connectors.
Normally the telco hands off a cable with a male end to me which I plug into my router's NM-1T3 female end. I never have to buy the cable just the expensive NM-1T3 card (surly the price must have dropped since the one's I bought in late 1990s for $20k each).
Looks like you need a female (ADC) to male (Cicso) 75-ohm coaxial cable.
I've honestly don't understand why the ADC equipment is not female, or why in 2013 they aren't giving you a Ethernet handoff :)

I figure someone here is familiar with this. I have never installed a new DS3 before and am unfamiliar with the cable ends and types for the patch.
Here is a picture of my demark extension: http://i.imgur.com/CN1vHnP.jpg
I need to connect this to a Cisco NM-1T3/E3.
Can someone point me to the right type of cable I'd need?
Thanks!
