+ on primary starts working in 9.0.

I have it on lots of systems.  Do you see the right calling number in the port status monitor or RTMT?

Have Unity Cxn 9.1.2 with E164 configured on Primary Extn. When someone presses their messages button Unity is not recognizing the extension. It works fine if I define the E164 extn as an alternate. (Leaving messages works fine in both cases.)

I thought starting with 8.6 E164 on primary was supported. Anyone having a similar issue?
