Look like power issue. 
You can check the status using command 
show chassis power-budget-statistics

in-case the module remains offline after 2+ power supplies are up, you can refer to below KB article to troubleshoot.

Might be power.  I've got two 208V's lit. Awaiting electrician on the rest.  Doing the request says the FPC is in transition.
