T-Mobile filed the following comment with the FCC regarding the June 15,

Stripping away the marketing and self-adulation, the following two 
sentences describe what happened
> Based on T-Mobile’s analysis, the cause of the outage was a leased fiber circuit failure from a third-party provider in the Southeast.  The built-in redundancy failed and resulted in an overload situation that was then compounded by an unforeseen defect in third-party software.
