Thanks to all, turns out to be an issue with a sub-tending carrier for L3. Still working to resolution.

I've found that increasing tone.dtmf.level on the Polycom phones from the default of "-15" to "-12" or "-9" often helps with problems where conference bridges, IVR systems, etc. aren't receiving some/all of the digits.

If that doesn't help, you will need to work with L3 support.  They have had numerous DTMF issues with their gateway vendor over the years, and you may be running into the next in a long line of bugs from them.
