Update on this incase anyone is interested.
I got this working on my Broadworks (R20sp1) http://xchange.broadsoft.com/php/xchange/node/449863
I added all of the Caribbean NPAs to the NDC table setting the Alternate Call Indicator to their ISO-3166 Alpha 2 code (2 letter country code). Now I can easily enable/disable the islands in the NANP through a Communications Barring Profile.
My country 1 NDC looks like:
Country Code From To Name Min STNC Length Max STNC Length NDC Length Zone Rate Center LCA Id Call Ind

We made a list in the dialplan that traps all calls to high-fraud areas. We made that list from various online sources regarding call fraud. Some of the Caribbean, Nigeria and other African regions, etc. The only complaint we had over the years was for one Caribbean area code that is high fraud, but low cost, so we enabled it.
We then have our carrier capped at something like 25 cents/min and a daily dollar cap.
I think we could share our list, however it was built many years ago and you probably want to re-research it yourself.

VoIP hacking is a way of life these days, I swear I think there is an Olympic sport for hacking now.
By default we block all international calls on users (011 dialed) and will enable using auth codes or outright if the customer insists and bears all potential costs. That has significantly reduced / eliminated international toll fraud on our switch. Now, we see fraud going to NANP (1+ dialed) high cost ratecenters/NPANXXs.
Is there a list of the high cost NPA-NXXs or should I just go through my upstream carriers rate deck and sort by $$/MOU?
Does anyone know how to mark the high cost NPA-NXXs in the Broadworks Network Server so the calls are marked as high cost / expensive? Then I can treat them as international and block or require an auth code.
