There are a few others I have seen; Voicemail port wizard failing and problems with updating Gateway information. Of course when these problems occur there is a TAC case that is created and it usually takes a awhile for us to notice the URL has an underscore. Most of the problems can be worked around by using the IP Address instead of the hostname in the URL. Also, an underscore is not supported by Cisco TAC.

interestingly enough, the underscore is heavily used in the active directory world. we made a conscious decision in our DNS team to allow these, because, well, without it, some active directory applications wouldn't work. our unix guru did some research and basically, as wes mentioned, strictly speaking it's not supported, but people have moved towards it. so as long as your internal DNS servers support it, then you should be ok. i think the danger is when you try to create dns entries with underscores that will hit the outside world.

underscore was not a valid character in the original dns rfc. you
will get mixed interoperability. safest option is to not use underscore.
