[cisco-voip] HTTP Status 404 - /ccmuser/

I believe CSCsv07585 can also cause this problem.  Make sure the user is
logging in with the correct case as specified in the enduser table if you
are running a version that does not contain the fix for CSCsv07585.

That's a third issue, good catch Ryan.    In our case, a majority of the
time these are just end users that would have never logged into ccmadmin.
