have a tac case open.

"Apparently the issue was on our Voice Gateway (which is H323). Cisco
suggested the following: 

"Add 'incoming called-number .' to one of the voip dial peers on the
gateway...dial-peer voice 20 or 30 voip...." 
"dial-peer 0 was trying to setup using g722"

CM can set this cause code but does not seem likely in this case.

peer switch says to use channel 23 and everything progresses fine.
peer switch says to cut through audio and immediately the call is
disconnected.

Just a guess - the DSP for channel23 either does not exist or is
terminally ill.  Do you have enough DSPs for the total number of DS0's?
Are you using Flex DSP allocation or static DSP allocation?
