Are you using Unity Server in clustering too? Issue one could be configuration or Exchange services issue. 
 
Did you try to open the user's mailbox via outlook web access and check for any e-mails (voice-mails actually) present there when they complained?
 
You may go to Unity>> Report section and run the report for subscribers. That will give you information about messages received by the complaining subscribers.
 
Issue 2#
If MWI is not lighting up all all, it could be configuration issue. Check for duplicate MWI on/off extensionsi CCM. 
 
MWI ports have access to subscriber's extension numbers
 
Ensure MWI on/off extensions are added correctly in Unity.
 
Also look at:
Subscribers > Subscriber Template > Messages Page
Use MWI for Message Notification

At times I've noted MWI issue with Unity which goes away with a Reboot, just in case if you can try..

Gregory,

If you haven't yet, look at the "Unity Architecture Overview" that you
can download from www.ciscounitytools.com. It will help you figure out
which part is broke. It does a good job of explaining what all the
moving parts do.

Off the top of my head, have you re-run the permissions wizard since
you added exchange servers and so forth? It sounds like a permission
problem or a communications problem w/ exchange to me for both #1 and
#2.
