For basic IPPA on 9.0(2), which you should validate first and then try One
Touch, it's at:

http://uccxserver:6293/ipphone/jsp/sciphonexml/IPAgentInitial.jsp

The output of the above on is:

<?xml version="1.0"
encoding="ISO-8859-1"?><CiscoIPPhoneInput><Title>Agent
Login</Title><Prompt>Enter agent
information.</Prompt><URL>http://10.1.1.30:6293/ipphone/jsp/sciphonexml/IPAgentLogin.jsp</URL><InputItem><DisplayName>Name</DisplayName><QueryStringParam>ID</QueryStringParam><InputFlags>A</InputFlags><DefaultValue/></InputItem><InputItem><DisplayName>Password</DisplayName><QueryStringParam>Pwd</QueryStringParam><InputFlags>AP</InputFlags><DefaultValue/></InputItem><InputItem><DisplayName>Extension</DisplayName><QueryStringParam>Ext</QueryStringParam><InputFlags>N</InputFlags><DefaultValue/></InputItem><SoftKeyItem><Name>Submit</Name><URL>SoftKey:Submit</URL><Position>1</Position></SoftKeyItem><SoftKeyItem><Name><<</Name><URL>SoftKey:<<</URL><Position>2</Position></SoftKeyItem><SoftKeyItem><Name>Exit</Name><URL>Init:Services</URL><Position>3</Position></SoftKeyItem></CiscoIPPhoneInput>

Which does correctly draw the login page on a phone.

If you have HA, make sure you are pointing to the master engine. Also, if
you have HA, you'll have to configure to phone services: one for each
server and have the Agent try one and then the other. It's pretty basic.

Once you validate that IPPA works, then moving to OneTouch takes three
steps, give or take:

   1. Change your service URL to:
   http://uccxserver:6293/ipphone/jsp/sciphonexml/IPAgentLogin.jsp
   2. Add three parameters (Case Matters):
      1. Ext
      2. ID
      3. Pwd
   3. Update the service on the phone and then enter the values for the
   three parameters
